Here is a report from Germany which considers COVID-19 to be a false alarm:

Obviously, for largely unknown reasons, the disease appears to be worse in some countries than in others.
Could it be that more regimented societies are better able to ward off such a pandemic? We may never know.

Hong Kong has had only 4 deaths. 4. Why is that? Here is one possible explanation from a Hong Kong resident:
But I think the main reason is because Hong Kong people, while they do not trust the words of their government, and still less those of the government over the boundary on the mainland, did not need to be persuaded to take the whole matter seriously. This made a big difference.

I urge you to read the entire letter. I agree with its author. Our leaders failed us big time, but they did not spread the disease - we did. It's time we acknowledged our role in this disaster.
Something similar may be the case in Japan. Like Italy they have an aging population, but far fewer cases and deaths. Is it because they took this whole thing seriously, while we did not?
